Bridges is a deeply-rooted organization with a long history of community service and an exciting new vision & mission. We began in 1980 as Hospice of Charleston – South Carolina’s 2nd home hospice provider. We then opened the Tri-County area’s first in-patient hospice facility in 2006. In 2008, facing dramatic shifts in the health care landscape, the Bridges’ Board of Directors decided to sell the direct patient care operation to Gentiva Health Services, a national leader in hospice care, and shifted our focus to advocate for end-of-life planning more broadly in our community as Hospice of Charleston Foundation. Then in 2013, we rebranded as Bridges for End-of-Life to better reflect our distinction from hospice care providers and our new focus. In 2016-17, Bridges completed a thorough strategic planning process to refine our mission. Bridges’ mission is to empower individuals & families to navigate together a broad spectrum of issues encountered before, during, & after the death of a loved one. We accomplish this by advocating for advance care planning before a health care crisis, by training health care professionals & family caregivers to support families during end of life, and by empowering children to heal, grow & thrive after the death of a loved one.
